If you want hits or signups only from particular countries, you can advertise on sites where you can chose where your visitors will come from.
Many PTR/PTC sites enable this...
I've found three Manual surfs with trgeted advertsing:
http://Clickthru.com
http://TrafficG.com and
http://EM-Traffic.com

You can surf to get credits , you can buy them (expensive) or you can find few referrals and get credits this way.

You mean - put a site with google ads in manual surf? But google ads probably wont be clicked there...?
There are so called PTP (Paid To Promote) links which some sites have. If you put such a PTP link in a traffic exchange you get paid 50c - $1/1000 valid hits. Since many times only hits from some countries are valid, it is good to use targeted advertising...

Yes, you can also use Google Adwords to do just that and even more. You can target people in certain countries, regions and even within specific miles from a certain location. Quite useful for some niches.

Google Adsense have high tech softwares to find the source of any traffic hitting their ads and although not clicked it effects their payperimpression rate. They do not allow traffic exchange programs strictly.
